Here&#8217;s what those are and the steps you follow:<\/span><br \/>\n<\/strong><\/h4>\n<ol>\n<li>\n<h4>Team to find student ineligible \u2013 Student no longer eligible for a plan<\/h4>\n<ol>\n<li>\n<h4>Notify district 504 coordinator assistant to move student to 3 yr re-eval process<\/h4>\n<\/li>\n<li>\n<h4>Hold a team meeting (including parents) to find student ineligible (negligible or mild\/mod).<\/h4>\n<\/li>\n<li>\n<h4>No need to fill in the accommodations section<\/h4>\n<\/li>\n<li>\n<h4>Send meeting packet &amp; have plan checked<\/h4>\n<\/li>\n<li>\n<h4>Synergy will ask you Y\/N exit from 504 process once you finalize the Eligibility section.<\/h4>\n<\/li>\n<li>\n<h4>Say Y and it\u2019s done.<\/h4>\n<\/li>\n<\/ol>\n<\/li>\n<li>\n<h4>Parents ask to terminate the plan &#8211; student is still eligible for a plan<\/h4>\n<ol>\n<li>\n<h4>This is a revocation<\/h4>\n<\/li>\n<li>\n<h4>Have parent send you a l<span style=\"text-decoration: underline\">etter<\/span> of revocation, <span style=\"text-decoration: underline\">signed<\/span> (not an email)<span style=\"text-decoration: underline\"><br \/>\n<\/span><\/h4>\n<ol>\n<li>\n<h4>It must include the wording \u201cWe are revoking all 504 services\u201d<\/h4>\n<\/li>\n<\/ol>\n<\/li>\n<li>\n<h4>Scan\/send to me<\/h4>\n<\/li>\n<li>\n<h4>I\u2019ll upload letter and remove student from the SE 504 process<\/h4>\n<\/li>\n<\/ol>\n<\/li>\n<\/ol>\n<h3><strong><span style=\"color: #008080\">A student can be eligible but no plan (or accommodations) is needed at this time. Here&#8217;s the steps to follow:<\/span><br \/>\n<\/strong><\/h3>\n<ol start=\"5\">\n<li>\n<h4>The first time the student is found eligible but accommodations are not needed at this time (is taking medication that helps, has a health plan with all accommodations needed, etc)<br \/>\nnotify district 504 coordinator assistant to move student to 3 yr re-eval mode<\/h4>\n<\/li>\n<li>\n<h4>Fill in all sections BUT in accommodations write: \u201cEligible but no plan\/accommodations at this time\u201d<\/h4>\n<\/li>\n<li>\n<h4>Send meeting packet &amp; have plan checked<\/h4>\n<\/li>\n<li>\n<h4>Continue to monitor student annually following the regular cycle of renewals\/3 yrs.
