504 information in Student Services Portal site<\/span><\/strong><\/h4>\n

Items include:  Teachers using TeacherVue<\/a> to see 504 accommodations; Bringing a student<\/a> into the SE side of Synergy, Workflow of forms<\/a> to fill when doing an initial, 3 yr or annual; Synergy forms examples<\/a>; Additional 504 forms<\/a>; and more.<\/p>\n\n\n

A goal to set for 2022 RE: Writing\/finalizing plans checked:<\/strong><\/span><\/h4>\n

Between now and December 16th have all your 2022 plans due written and finalized.<\/p>\n\n\n

\n\n\n

Accommodations<\/strong><\/span><\/h3>\n

Wording to avoid using in accommodations:<\/strong>
     Jamie can go to the (nurse\/counselor) office as needed
     (Nurse\/counselor) will be available for XYZ situation<\/h4>\n

Instead, use generic wording like:<\/strong>
     Follow the attached Health (etc) plan for emergency action plans, details, and additional protocols   
     Ability to access an approved preferred person\/safe adult\/space to decompress, etc.<\/h4>\n\n\n

Student coming in with a plan from out of state\/district. <\/span>
Which process do you bring them in under?<\/span><\/h4>\n

As a 504 Initial.<\/strong><\/h4>\n\n\n
\n\n\n

Four main points to remember through the year:<\/span><\/h3>\n
    \n
  1. \n

    ALL HS senior plans must be renewed throughout the school year<\/h4>\n<\/li>\n
  2. \n

    All plans (including seniors) must be completed by May 31st. No Exceptions. (unless a new 504 plan comes in)<\/h4>\n
      \n
    1. \n

      Plan accordingly if you are involved in EOY events. Write due 504 plans earlier.<\/h4>\n<\/li>\n<\/ol>\n<\/li>\n
    2. \n

      All concurrent plans must be renewed until the IEP icon is on in Synergy<\/h4>\n<\/li>\n
    3. \n

      Do not finalize a 504 plan until it has been checked by the district 504 support person.<\/h4>\n<\/li>\n<\/ol>","protected":false},"excerpt":{"rendered":"
