ODE 504 Training —  SAVE THE DATE<\/strong><\/span><\/h3>\n

\u201c504 Coordinators, we have arranged for a training by the Oregon Department of Education.
This training will take place on January 18th<\/sup> from 8-12<\/strong>
at the Education Center Room 314 (Board Room)<\/strong>
Please know that this training is for any administrator, or counselor that is currently involved in 504.
<\/span><\/em>If you are new to 504 I would consider this training to be required<\/span>.<\/strong>
If you are a veteran I would strongly encourage you to attend.<\/strong>
This is an opportunity for you to learn and ask questions about 504 from our partners at ODE.<\/em><\/p>\n

Please go into Performance Matters and sign up, so we have an accurate count of attendees.<\/strong><\/em><\/p>\n

For Initials and 3 yr re-eval.
We’re going back to you collecting the signature for the Synergy Section “Notice to Conduct a 504 Evaluation Form”

If the meeting is in person:<\/span><\/strong><\/h4>\n
\n
    \n
  1. \n

    enter the information into this form PRIOR to the meeting
    <\/span><\/h4>\n<\/li>\n
  2. \n

    print it out and bring it to the meeting<\/span><\/h4>\n<\/li>\n
  3. \n

    have parent sign<\/span><\/h4>\n<\/li>\n
  4. \n

    scan it to me at the end of the meeting along with the 504 Meeting Packet
    <\/span><\/h4>\n<\/li>\n<\/ol>\n

    If the meeting is over the phone, virtual, etc:<\/strong><\/h4>\n

    1. \n

      You’ll send the filled form home to the parent to sign<\/h4>\n

      IF THAT FAILS, THEN:<\/strong><\/span><\/h4>\n

      2. I will send it out via Adobe Sign electronically<\/h4>\n<\/li>\n<\/ol>\n
