(in process)<\/li>\n\n\n\n<li>Copies of the release form will be sent to&nbsp;<strong>Mosaic, Summit Medical Group, and COPA when the new Immediate Temporary Accommodations Plan is finalized (hopefully in the next few weeks).&nbsp;<\/strong><\/li>\n\n\n\n<li><strong>Coaches<\/strong>&nbsp;will communicate this new process to athletes and families.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Alternate Forms<\/strong>\n<ul class=\"wp-block-list\">\n<li>Any documentation&nbsp;<strong>other than the BLS Medical Release Form<\/strong>&nbsp;will be reviewed&nbsp;<strong>on a case-by-case basis<\/strong>.<\/li>\n\n\n\n<li><strong>OSAA Medical Release Forms<\/strong>&nbsp;will also be reviewed; however, acceptance will depend on which box is checked.
