This is a PDF version<\/a> of the form that can be used to collect the info before you\u2019re ready to submit the online form. Submitting this online form is how you get your $1,000 start-up funds and Sources of Strength materials from Matchstick. <\/p>\n\n\n\n You can consider the May 23rd<\/sup> as your Adult Advisor training and we can make plans for when your Fall 2023 Teen Leader training will be done at the May 23rd<\/sup> Adult Advisor training. <\/p>\n\n\n\n Schools that are ready to start Sources of Strength sooner than the beginning of next school year can proceed on their own with submitting the online form to Matchstick (see the link below) and can set up their own Adult Advisor and Teen Leader trainings directly with Matchstick. Your school\u2019s Public Health Specialist can help.
