In line with our protocol, there is a spot to document the lethal means conversation with parent\/caregiver at the top of the form, as well as a reminder at the bottom of the form that parent notification is required each time a screen is given, even when “no” is the last answer on the C-SSRS.<\/p>\n\n\n\n Do you want to download, open, and edit the C-SSRS?
