To help with that process, we created the following example forms to get&nbsp;<a href=\"https:\/\/docs.google.com\/forms\/d\/e\/1FAIpQLSeFXa6FSEGC4Y0y9gAdffRB6YZJI_DWegOV-nEJpO0YmGN7uA\/viewform?usp=sharing\">digital<\/a>&nbsp;or&nbsp;<a href=\"https:\/\/docs.google.com\/document\/d\/1EaUR9W0yPPiC9Cotm3pQnmZOxt-4fAgPn7z7A3JeceI\/edit?usp=sharing\">written<\/a>&nbsp;input.
