Year End Checklist

Tamarack Ctr., Transition Co-op, Nurses, Psychs, APE and SPO Admin only:
___ June 17, Receipts for petty cash reimbursements for supplies and mileage/travel reimbursements through the end of the school year are due to Heidi Hanson

Certificated only:
___ All leave for June is to be phoned to Heidi DAILY in addition to being reported on a leave record form that is due the day you return from being out (or before you go if it’s for personal leave already requested). Heidi’s ext. #: 1061

___ June 11, Complete and finalize Prior Written Notices in Synergy for students:
1. Graduating or transitioning to T.C.: Prior Written Notice of Graduation or Post-Secondary. Summary of Performance must also be completed prior to finalizing the PWN of Graduation
2. Who have moved: Prior Written Notice of Moved, Continuing Education
3. Who have dropped out: Prior Written Notice of Non-Attendance/Drop out

___ June 11, IEPs and all related IEP documentation to your Records Clerk for review
If after June 11th, you are responsible for finalizing all documents, uploading and attaching to the correct process in Synergy, and getting paperwork to parents. SPED teachers and SLPs will have to have an “all complete” email from Shannon Watters to check out with site Principal. Progress reports and all paperwork will need to be completed.

___ June 11, 2021-2022 electronic transportation requests need to be completed for all students

___ June 17, Return any materials/electronics borrowed from the Special Programs Office
(iPads for student medical tracking, external dvd players, etc. – NOT sped laptops or district iPads)

___ June 11, Return all Shared Literacy Kits to Shannon through inter-district mail

___ June 17,
SLPs – finish inputting Medicaid logs into ORMED
Nurses – scan or mail your final fee tickets
Don’t forget to sign them electronically or on paper!

___ June 17, SLPs – bring your audiometers to room 317 in the Education Center for summer calibrating. Please give them to Denise or Heidi to check out

___ June 17, ESY student materials are now at Special Programs (room 317) c/o Denise Sevigny

___ June 18, Paperwork Checklist: Progress Reports, PWNs, and IEP paperwork must be completed. You will get an email from Shannon Watters to get the ok to check out at your site

___ June 18, All high cost forms must be submitted to Heidi
(NOT to be confused with staffing needs assessments)

___ June 18, Continue inputting Medicaid Activity Logs (fee tickets) until your last contract day. If you have questions, contact Heidi Hanson. ext. # 1061

___ June 18, Permanent addresses. Send an email to Heidi ONLY if your permanent address or contact information has changed [email protected]

___ June 22, All Restraint and Seclusions that happen in the last week of school need to be turned into Krista Baker by noon on June 22nd


The laptop/iPad EOY jingle

ALL Special Education Itinerant Staff:
      – Psychs, Student Success Coaches and SEL Director
      – Certified/Classified: SPO staff, Nurses, Speech, APE, SPSecondary_Ed Ctr, and T.C.
If you are moving to a new location:  take your laptop and district issued iPad with you
If you are leaving the district:  bring your laptop and district issued iPad(s) to Denise Sevigny in the Ed Ctr TLC Office 314.

ALL Special Education School-based Certified Staff:
If you are moving to a new location or leaving the district
     – Leave your laptop with your school. The new school will provide you another.
     – Bring your district issued iPad to your new setting.
