Please turn in all ESY parent/guardian consent paperwork to Magadalyn Hasse by April 28.
Prior to having parents/guardians sign, please check the box as to whether the child on you caseload will be attending:
- 8:30-10:00 ESY session ( 90 minutes or less of SDI per day, as designated on their IEP)
- 8:30-11:30 ESY session (more than 90 minutes of SDI per day, as designated on their IEP).
- GRO/EXCEL (behavior, emotional, and/or social skills goals ONLY, as designated on their IEP)
- If a student qualifies for GRO/EXCEL, please have the parent/guardian complete the GRO/EXCEL consent form in addition to the ESY parent/guardian consent.

*Please note – anything that is not on the district-approved curriculum list (attached to the Google form), will need to be approved by Student Services administration.
