A reminder for collecting signatures on the “504 Consent to Evaluate” section.
If parents don’t have email ability and you need to or you’d rather collect the signature on your own – that is fine. Just let me know.
- Counselors/coordinators fill in an initial or 3 yr re-eval
- You let parents know they’ll be receiving an email asking for their signature on the Consent to Evaluate (from Adobe Sign)
- You send me the packet after the meeting when all electronic sections are filled
- I check the plan, email you with additions, if any, and pull the electronic consent
- I email the consent to a parent via Adobe Sign
- Once signed, it comes back to me; I upload it
- I let you know the plan is ready to be finalized
