Need to revise a recently written 504 plan? – after a meeting/discussion with parent(s) to adjust/add accommodations
Check to see if:
1. The 504 is in the annual process. All is good. You create an annual with the updated/additional accommodations included. (An annual has two sections showing to fill – Section 1, Meeting Notice and Section 2, Accommodations).
2. The plan is in a 3 yr renew process – Do not write a 3 yr when all you need to do is update/add new accommodations to a recently written plan.
First, contact your district’s 504 coordinator assistant to move the student’s 504 process from the 3 yr to the annual. Once that is done, you can write your annual with the updates/additions.
(A 3 yr has 4 sections showing to fill – Section 1, Meeting Notice; Section 2, Notice to Conduct a 504 Evaluation; Section 3, Eligibility and Section 4, Accommodations).
A reminder as we move into the new year and into the rapidly approaching EOY.

All plans must be completed by May 31st. (unless an initial). Plan accordingly so that if you have heavy EOY duties, you can get these done by May 31st.

All Concurrent plans must be renewed until the IEP is ‘active’ (icon is on)

All HS senior plans must be renewed throughout the whole school year
