Folks, here’s a link to our new portal site’s 504 information ‘folder’
It’s worth taking a look as it has documents that address many of the questions you send my way.
For instance:  Teachers using TeacherVue to see 504 accommodations; Bringing a student into the SE side of Synergy, Workflow of forms to fill when doing an initial, 3 yr or annual; Synergy forms examplesAdditional 504 forms; and more.

Four main points to remember when writing 504 plans:

  1.  ALL HS senior plans must be renewed throughout the school year

  2.  All plans (including seniors) must be completed by May 31st. No   Exceptions. (unless a new 504 plan comes in)
    1. Plan accordingly if you are involved in EOY events.
      Write due 504 plans earlier.

  3.  All concurrent plans must be renewed until the IEP icon is on in Synergy

  4.  Please do not finalize a 504 plan until it has been checked by the district 504 support person.

Tidbits:

  1. Student coming in with a plan from out of state/district, which process do you bring them in under?   As a 504 Initial.

  2. Your 504 meeting is finalized. Who sends parent(s) the completed 504 packet?     You do by:
      • Go to Historical docs (once plan has been checked by district 504 support person and finalized)
      • Click on the little yellow folder in front of your finalized 504 plan
      • Click on ‘Packet Choices’ at the top of the window that pops up
      • Click on ‘Print selected files as one’
      • A document will be created and will open
      • Save this document to your desktop to send to parents
