There are two scenarios that can happen which will allow a student to be exited from the 504 process. Here’s what those are and the steps you follow:

  1. Team to find student ineligible – Student no longer eligible for a plan

    1. Notify district 504 coordinator assistant to move student to 3 yr re-eval process

    2. Hold a team meeting (including parents) to find student ineligible (negligible or mild/mod).

    3. No need to fill in the accommodations section

    4. Send meeting packet & have plan checked

    5. Synergy will ask you Y/N exit from 504 process once you finalize the Eligibility section.

    6. Say Y and it’s done.

  2. Parents ask to terminate the plan – student is still eligible for a plan

    1. This is a revocation

    2. Have parent send you a letter of revocation, signed (not an email)

      1. It must include the wording “We are revoking all 504 services”

    3. Scan/send to me

    4. I’ll upload letter and remove student from the SE 504 process

A student can be eligible but no plan (or accommodations) is needed at this time. Here’s the steps to follow:

  1. The first time the student is found eligible but accommodations are not needed at this time (is taking medication that helps, has a health plan with all accommodations needed, etc)
    notify district 504 coordinator assistant to move student to 3 yr re-eval mode

  2. Fill in all sections BUT in accommodations write: “Eligible but no plan/accommodations at this time”

  3. Send meeting packet & have plan checked

  4. Continue to monitor student annually following the regular cycle of renewals/3 yrs. Continue writing

    “Eligible but no plan/accommodations at this time” in accommodations
