Refresher – Bringing a student into the 504 process
A reminder on signatures on “Notice to Conduct a 504 Evaluation” form
Remember that whether you are holding a 504 3 yr or Initial meeting in person, by phone or by computer; it is now your responsibility to get the Notice to Conduct a 504 Evaluation form signed by the parent.
At the meeting if in person,
Asking parent to come in and sign if virtual – or sending home to have them sign – or…
Asking for an Adobe Sign invite to go out for an electronic signature is your last ditch request saved for those families who are not signing after many attempts.
