504 Consent to Evaluate – filling in dates, consent yay/nay
When filling in a plan during your meeting, do you fill or don’t you fill in the consent dates/Yes No boxes if you are sending the consent electronically after the meeting?

Bend-La Pine Schools’ Health Consultants Team had a meeting with the district where they shared that they are seeing a dramatic rise in eating disorders.

Please be aware and don’t be afraid to ask boys and girls questions:

Look at behaviors and self-talk, not size.

Ask students to see their pediatrician and talk to their doctor.

Student is eligible for a 504 plan but has a health protocol in place that lists all the accommodations needed. Do you need to write a plan?
Yes.
                                       Health accommodations only:
Fill in all docs (whether doing Initial, renewal, 3 yr re-eval)
Under Accommodations write in “Student eligible but no plan due to Health Protocol. See Health Protocol for accommodations.”
No other accommodations should be typed into the plan as this will mean there is a plan in addition to a protocol.
                                    Educational accommodations too?
You write a full plan.



You have extra documents on a student:
          1. (protocols, evals, behavior plans, doctor’s notes, etc).
          2. Please send them to me to upload.
          3. Please DO NOT upload them into a plan.

October 13, 2021 from 8a – 12:30p – (includes two 15 minute breaks).

You will need to register in Performance Matters.
Go to the Portal, under quick links click on Performance Matters, employees.
Click on My Courses and enter the following course #:  10285


Student leaves the district. Still on your portfolio. Annoying!!
When a student with a 504 plan leaves the district, you’ll continue to see them in your portfolio in (parenthesis). They will stay as a reminder should they return to BLPS – and yes, they will show as red due, due, due.
You can change your Synergy view to Active Only so that you no longer see them but that’s your only option..

Need to add an addendum to a 504 plan?
If you need to add an addendum to a plan: (for instance, an additional accommodation or a missed participant)
You’ll have to write a new plan (renewal) – which includes a Meeting Packet & plan check
Use the same date of the last plan written so it doesn’t change your dates of renewal

Your 504 meeting is done.
Who sends parent(s) the completed 504 packet?

  1. You do
    1. Go to Historical docs (once the plan has been checked by me and finalized)
    2. Click on the little yellow folder in front of your finalized 504 plan
    3. Click on ‘Packet Choices’ at the top of the window that pops up
    4. Click on ‘Print selected files as one’
    5. A document will be created and will open
    6. Save this document to your desktop to send to parents

A reminder for collecting signatures on the “504 Consent to Evaluate” section.

To help out with your load, here is an available process for getting the Consent to Evaluate signed: (However, if parents don’t have email ability and you need to or you’d rather collect the signature on your own – no worries! That is just fine. Just let me know you’ve done so.)

  • Counselors/coordinators fill in an initial or 3 yr re-eval
  • You let parents know they’ll be receiving an email asking for their signature on the Consent to Evaluate (from Adobe Sign)
  • You send me the packet after the meeting when all electronic sections are filled
  • I check the plan, email you with additions, if any, and pull the electronic consent
  • I email the consent to a parent via Adobe Sign
  • Once signed, it comes back to me; I upload it
  • I let you know the plan is ready to be finalized
