New 504 Coordinators/counselors, this is a mandatory meeting
Existing 504 Coordinators/counselors, you are highly encouraged to attend

Presenter – Winston Cornwall, ODE’s 504 Specialist
October 13, 2021 from 8a – 12:30p – (includes two 15 minute breaks).
Trinity Episcopal Church – Brooks Room

You will need to register in Performance Matters.
Go to the Portal, under quick links click on Performance Matters, employees.
Click on My Courses and enter the following course #:  10285

Currently the training is planned for in-person, but this could change


Process for students exiting from a 504

There are 2 ways a student can exit from a 504:

  • Team to find student ineligible:
    1. Notify district 504 coordinator assistant to move student to 3 yr re-eval process
    1. Hold a team meeting (including parents) to find student ineligible (mild/mod).
    1. No need to fill in the accommodations section
    1. Send meeting packet & have plan checked
    1. Synergy will ask you Y/N exit from 504 process once you finalize the Eligibility section.
    1. Say Y and it’s done.
  • Parents ask to terminate the plan – student is still eligible for a plan
    1. This is a revocation
    1. Have parent send you a letter of revocation, signed
      1. It must include the wording “We are revoking all 504 services”
    1. Scan/send to me
    1. I’ll upload letter and remove student from the SE 504 process

Non-exiting option:

Eligible but no plan (or accommodations)

  1. Notify district 504 coordinator assistant to move student to 3 yr re-eval mode
  2. Fill in all sections AND in accommodations write:  “Eligible but no plan/accommodations at this time”
  3. Send meeting packet & have plan checked
  4. Continue to monitor student annually

Process for incoming out-of-state/district 504 plans – if taking time to collect data
1. Send the incoming 504 plan to Denise to upload
2. Notify teachers of the 504 accommodations if not immediately holding a meeting
3. Observe the student for no longer than one month
4. Bring the student into SE Synergy as an INITIAL (New 504 Evaluation)
5. Write the Bend-La Pine plan
6. Send meeting packet to Denise for a plan check

If you are writing a plan immediately, all you need to do is 4, 5 & 6

Synergy Canvas Course
Below, click on the ‘504 Introduction flier’ to see how to access a Synergy Canvas course for 504 coordinators. This course is self-paced through Canvas; and you’ll need to sign up via performance matters. 

New 504 Coordinators – All new 504 coordinators MUST complete this course before having your first 504 meeting. All 504’s are to be completed in Synergy.
 
Returning 504 Coordinators – You can work on this course as a review. Each section can be completed as a standalone, so you can go through only the parts that you need a refresher on.

504 Blog 6/1/2021

If you have a plan that is due, you’ve reached out to parents multiple times to schedule a meeting, and they will not respond – it is now time for you to please do the following:
1. schedule the meeting with your team
2. document your attempts to communicate – date, method – in the meeting notes
3. write the plan, send the meeting packet to me for a plan check
3. and send parents a copy of the finalized plan

Do this ESPECIALLY if it is an initial or 3yr as I need time to get parent signature before everyone disappears for vacations.
