When bringing in a new student from General Ed Synergy PLEASE remember to follow these steps: New 504 plan process.


To find a student’s current electronic 504 plan

Remember, you will no longer find current student 504 plans under the documents tab in GenEd (SIS) Synergy.
Instead…..
– Click on the student’s name in your Portfolio
– Click on Historical Docs
– And Click Here to understand what you are seeing

Last reminder of this 504 training with Winston Cornwall.
SAVE THE DATE: 
Our Fall 504 training will be November 4th, 8a-12:30p (includes two 15 minute breaks). You need to register in Performance Matters, Fall 2020-504 Training
The training will be via webex, in Josh’s room:  https://bls.webex.com/meet/josh.marks
We will have Winston Cornwall from ODE here to share information about Section 504 and answer questions. This should be considered a required training.
It will include some question/answer opportunities, some breakouts and polling, as well as a wide range of topics, some of which are listed below:

A reminder for collecting signatures on the “504 Consent to Evaluate” section.

If parents don’t have email ability and you need to or you’d rather collect the signature on your own – that is fine. Just let me know.

  • Counselors/coordinators fill in an initial or 3 yr re-eval
  • You let parents know they’ll be receiving an email asking for their signature on the Consent to Evaluate (from Adobe Sign)
  • You send me the packet after the meeting when all electronic sections are filled
  • I check the plan, email you with additions, if any, and pull the electronic consent
  • I email the consent to a parent via Adobe Sign
  • Once signed, it comes back to me; I upload it
  • I let you know the plan is ready to be finalized

1. ALL high school senior plans MUST be renewed throughout the school year.

2. Starting this school year, ALL plans must be completed by May 30th. That includes plans due in June. The only exception is an initial that needs to be done.
       Please plan accordingly to get all completed in May

3. All concurrent plans must be renewed until the IEP is active (IEP icon is on, plan is in use)

Three main things to remember for this school year on:
1. All HS Senior
plans MUST be renewed throughout the school year. No exceptions
2.
As of this school year, ALL plans must be completed by May 30th. Plan accordingly if you have end of the year obligations/events you need to participate in

            One exception: You have an initial to create.
3. All concurrent plans must be renewed until the IEP is active (IEP icon is on, plan is being used)

The Re-evaluation check

Make sure to check the date of your student’s review/re-evaluation each time you start to do a plan. Why?
Synergy may not automatically move the session into the re-evaluation process if the dates are close to one another (or the re-eval was due earlier in the year).
(It’s looking pretty rare with our new setting, but be prepared by checking dates – in yellow)

If you end up doing a review when a re-eval is the one really due, the re-eval will roll into place immediately after finalizing the review and your portfolio will continue to show the student in red even though you just had a meeting for the renewal. You’ll then have to do the re-eval session.
