What to do with COIC students on a 504 plan.

You’ve had a student move to COIC.

Do you continue to serve this student by renewing the 504 plan annually?
Yes. The 504 plan stays in place and is renewed annually unless you find the student ineligible. You will need to ‘share’ the 504 with COIC.

Otherwise, here are a few reminders!


Holding a 504 plan meeting without parent(s) present
If after multiple attempts to reach a parent(s) to hold a 504 annual/re-eval meeting the parents have never contacted you, you are able to hold this meeting with your team without parent participation.
1. Document your attempts to email, call, text the parents in the notes page of the meeting packet. Remember to add the dates of these 3 plus attempts.
2. Once the team has completed the plan, you’ve had the plan checked, and have finalized it – send the plan packet to the parent(s).

Please keep in mind:
1. ALL high school senior plans MUST be renewed throughout the school year.

2. Starting this school year, ALL plans must be completed by May 30th. That includes plans due in June. The only exception is an initial that needs to be done.
       Please plan accordingly to get all completed in May

3. All concurrent plans must be renewed until the IEP is active (IEP icon is on, plan is in use)
