NEW DISTRICT IEP INPUT FORM

From the desk of Colleen Funderburg

We are excited to launch the new IEP input form districtwide. This form will allow input to be collected from parents/guardians, Ged Ed Teachers, EA’s and other staff/team members.

The form will be included in all IEP meeting invitations created by the records clerks with a link that can be opened directly from the event in your calendar. Parents will also be sent the link to the form with any meeting notice sent by records clerks. (We highly recommend using the meeting request form to schedule and create your meeting notices)

Reminder – EA consultation prior to annual IEPs:

This fall, we shared important aspects about SB 756, which indicates that EAs must have an opportunity to provide input and be consulted with prior to a student’s IEP Meetings. To help with that process, we created the following example forms to get digital or written input. If you use either of these forms, please make a copy!

If you have any questions, please reach out to the student services team. “
