504 Tips

As we come to the end of May, I want to thank all of your for your hard work in watching over our students with a 504 plan. Keeping them updated, meeting deadlines, checking that accommodations are being used, and more. You are much appreciated!

Remember –
Always have the plan checked BEFORE finalizing by sending your 504 meeting packet to me once you have filled in all the sections and the meeting has been held.

I will send consent to evaluate docs to parents electronically for their signature – Initials and renewals – unless you are holding a meeting live.

Tasks to complete for the end of the school year

1. Please check your portfolio for:
   1. Plans to fill and have checked by May 31st
   2. Plans filled, checked and needing to be finalized
2. Routine check of the Student 504 report list in Google docs:
   – Verify students who are graduating
   – Verify the transitioning school listed for students moving up
   – Verify the students transitioning to your school (you’ll see them at the very bottom of your list unless there are none transitioning in)
3. Send Denise an email if there is:
      – A student still on the Student 504 report who has left the district/school or whose plan ended
      – There is a student on a 504 plan who is missing from the Student 504 Report
      – Transitioning student information is incorrect (going to a different school)

1. Plans needing reviews by the end of the school year:
   – Please finish all reviews by May 31st.
   – If unable to reach parents after multiple attempts, document the attempts, hold the meeting with a school team and let parents know of findings.
2. Review due on a transitioning student? PLEASE contact MS/HS 504 Coordinator to find out their preference:
   – Plan stays due till in new setting   —–OR—–
   – You renew (with their input for new setting).
3. HS Graduating:
   – Plan is due, you need to do a renewal in case they want to carry it to college.
   – They will be rolled out of Synergy automatically over the summer.
   – You do NOT have to do anything to exit a graduating HS student

504 student leaves the district but is still in your portfolio. 
WHY?!

All those red circles and squares shouting DUE! DUE! DUE! but the student has left the district! Why?
All in the hopes the student will return, and it’ll be noted s/he/they were on a 504 plan.

Synergy continues to carry a student that has left/been dropped through the remaining school year. Unfortunately, that means that student continues to show in the portfolio (in parenthesis to let you know the student is gone) with red ‘due’ circles and squares showing.

You can ignore all those red symbols and ignore students in parenthesis in your portfolio
or
You can ‘hide’ them by choosing Active only in Synergy (up in the right corner by your name) but then you have to remember to change it back after viewing your portfolio.

By the start of next school year, that student will be gone from your portfolio (as long as they haven’t return).

Exiting a student from 504 Services –

1. Team to find student ineligible:
   1. Notify me to move student to 3 yr re-eval process
   2. Hold a team meeting (including parents) to find student ineligible (mild/mod).
   3. No need to fill in the accommodations section
   4. Send meeting packet & have plan checked
   5. Synergy will ask you Y/N exit from 504 process once you finalize the Eligibility section.
   6. Say Y and it’s done.
2. Parents ask to terminate the plan – student is still eligible for a plan
   1. This is a revocation
   2. Parent sends you a letter of revocation, signed
      1. It must include the wording “We are revoking all 504 services”
   3. Scan/send to Denise Sevigny
   4. I’ll upload letter and remove student from the SE 504 process

Eligible but no plan (or accommodations)

1. Notify me to move student to 3 yr re-eval mode
2. Fill in all sections BUT in accommodations write, eligible but no plan/accommodations at this time
3. Send meeting packet & have plan checked
4. Continue to monitor student annually
5. If you already established ‘eligible but no plan’ in previous 3 yr and are renewing with same status, doing a renewal and writing in accommodations ‘eligible but no plan/accommodations’ works

   – If student shows up at your school at the start of the new school year instead, a renewal needs to be done for the student’s current setting (your school)

1. Plans ending (parent/student revoking services, impairments were temporary)

Revocation – send me the parent letter/email stating they are revoking 504 services. I will exit student from SE 504.

504 services no longer needed (temporary impairment) email me to roll student into the 3 yr re-eval process so that you are able to find the student no longer eligible.
– Synergy will exit the student from SE 504.

All of you are doing GREAT at completing your renewals and re-evals for May 31st.
