Tasks to complete for the end of the school year

  1. Routine check of the Student 504 report list in Google docs:
    – Verify students who are graduating
    – Verify the transitioning school listed for students moving up
    – Verify the students transitioning to your school (you’ll see them at the very bottom of your list)
    – Send Denise an email if there is:
         – A student still on the list who has left the district/school or whose plan ended
         – There is a student on a 504 plan who is missing from the list
  1. Plans needing reviews by the end of the school year:
    – Please finish all reviews by May 31st.
    – If unable to reach parents after multiple attempts, document the attempts, hold the meeting with a school team and let parents know of findings.
     – Review due on a transitioning student? PLEASE contact MS/HS 504 Coordinator to find out their preference:
         – Plan stays due till in new setting   —–OR—–
         – You renew (with their input for new setting).
    – HS Graduating:
         – Plan is due, you need to do a renewal in case they want to carry it to college.
         – They will be rolled out of Synergy automatically over the summer.
         – You do NOT have to do anything to exit a HS student
    – BLPOnline full time:
         – You renew for the BLPOnline setting. Call Michelle to discuss.
         – If student shows up at your school at the start of the new school year instead, a renewal needs to be done for the student’s current setting (your school)
  1. Plans ending (parent/student revoking services, impairments were temporary)

Revocation – send me the parent letter/email stating they are revoking 504 services. I will exit student from SE 504.

504 services no longer needed (temporary impairment) email me to roll student into the 3 yr re-eval process so that you are able to find the student no longer eligible.
     – Synergy will exit the student from SE 504.

FYI.
I am continuing to collect signatures on the Consent to Evaluate Form for 3 yr re-evals and initials, electronically.
If you would like to collect the parent’s signature on the consent, great, but know that you don’t have to – unless parents do not have the electronics to sign via the computer.

504 Blog 3/16/2021

Since we continue to be in “COVID” mode and meetings continue to be held remotely, the only document requiring a signature when writing a 504 plan, is the “Consent to Evaluate” document that comes up when writing an initial or 3 yr re-eval.
For 504 Meeting Packet’s attendance page, all you need is a note stating something to the effect of “no signatures due to COVID”

504 Blog 3/9/2021

Student with a 504 is moving to an IEP. Do you need to do anything?
     – All you need to do is notify Denise that student is moving to an IEP.
Do note – the student will NOT be exited from 504 in Synergy until the IEP icon is up.
Why? Should the need for disciplinary action arise, all school staff will continue to see the 504 icon and know that the student has an impairment/accommodations.

504 Blog 2/25/2021

Whether writing an Initial, 3 yr review or an Annual, please remember:
– Do NOT finalize the plan until I have emailed you stating the plan is ready to be finalized.
Instead,
– Send me the Meeting Packet (attendance and notes) 
                   – and, if you have it –
– The signed Consent to Evaluate. (If you do not have this signed, that’s ok. I’ve been collecting the electronic signatures.)


Saving a 504 packet for parents after you:
1. Sent in Meeting Packet for plan check
2. Check done, electronic signature collected for Consent to Evaluate (if needed), documents were uploaded under the 504 sections tab
3. Email sent to you to finalize the plan
4. Plan finalized
– Saving the parent packet is easy in this scenario.
