Student on a 504 plan switches
mid-year to another school

To make sure student isn’t lost in the move, please do the following:

– If you know student is moving to another in-district school,          please notify the 504 coordinator at the new setting,    and…..

– Add them to the student’s team, remove yourself,     or…..

– Let me, Denise, know and I’ll get the above done

NOTE:  This does NOT apply to BLPOnline move. BLPOnline students with a 504 plan are still yours to case manage.

504 Blog 2/5/2021

For those of you who enjoy a video as a learning tool on:

Writing accommodations – in regards to students with a medical issue such as diabetes, seizures, etc.
Keep in mind:
A 504 plan is for educational related accommodations (Seating, lighting, adjustment of time for testing/homework, communicating with parents, small units of direction, etc.)

A Health plan is for the medical accommodations (allowing a student to take a fast acting sugar dose where they are, calling the Health Office if a student starts seizing, calling 911, parent or doctor if student is stung by a hornet/bee, etc.)

An example accommodation in a 504 plan that points to the health plan accommodations would be:  “Please reference the student’s diabetic health protocol and follow”

Reminders:

CDL and BLPO students will continue to be case managed by their home school.


A few steps that will need to happen for a smooth transition into 2021-2022 school year

1. All plans will be reviewed and finalized by May 31, 2021 (Initials being the
    exception, of course).
    Please work on getting reviews to me over a spread of time rather than in one large
    clump on May 31st.

2. If you have a student transitioning into MS or HS with a plan coming due in April/May,
please:
          A. Contact the MS/HS 504 coordinator and review plan for new setting together
or       B. Set an agreement with the MS/HS coordinator that plan review will occur at
               the new setting. If that is the case, please let me know.

3. Graduating students
          A. If student has a review coming due, it needs to be done.
          B. Synergy will exit the student over the summer. You do not need to do anything.

4. Full time BLPOnline students with a review coming due at EOY
          A. Review for the BLPOnline setting. Contact Michelle Oliver to set up a meeting
          B. If, in the new school year, student stays at your school, a review needs to
              be done for the student’s current setting
