Diploma Consent Forms for High School
The ICCL counseling team has worked on Diploma Change Consent Forms for students who are considering moving to an Honors, Standard and Modified Diploma. Here is a folder with the forms that we will use from this point forward. This form is lives within the BLS Counseling Drive for counselors to use as they are the leaders in conversations with students/families in regards to diploma change and graduation plans.
In this folder, there is a consent form for Oregon Standard Diploma, Honors Diploma and Modified Diploma. When making a diploma change it is best practice that all shareholders meet and are aware of the change (student, counselor, parent/caregiver). The administrator should be included in the process for switching from a BLS Academic Diploma to an Oregon Standard or Modified Diploma. Additionally, for all these diploma type changes, the form should be filled out and uploaded into the “Documents” tab in Synergy as it is a part of the student’s educational record. The school registrar has access to upload forms in documents.
(Modified Diplomas are not a common diploma type change and are most often seen with qualified students receiving special education services and the process lives with the case manager. However, there have been instances where general education students can qualify for a modified diploma based on data/evaluation. For general education students, counselors generally consult with administrators and Kay Ann Wells to talk through the process for modified diploma/curriculum, before presenting this option to change a student from BLS Academic to an Oregon modified diploma).
Use of Time Coming Up in April
We will send a reminder email closer to the dates, but wanted to get it on calendars as well. We will be doing a K-12 Use of Time Analysis April 3-7 and April 17-21. I sent out a calendar invite as well, please let me know if you didn’t get it!
Here is a link the Google Folder. In it you will find a folder for each school and “Use of Time Sheets” for each counselor at that school. There is one for April 3-7 and another for April 17-21.
The Use of Time Calculators are really easy to use, just put an X in the correct box for how you spent your time during that 15 minute period for each day.
